Please see the information below on how to make professional referrals into sexual health services.

Safeguarding Referrals

If you are a professional working with children and/or adults identified as vulnerable, you can make a referral into sexual health services by downloading the form below.

Please send the completed form via email to: LLRHealthAdvisingTeam@mpft.nhs.uk 

Safeguarding Referral Form

General Practice Referrals

You can make a General Practice referral for specific conditions via PRISM (Sexual Healthcare (Complex) tab)

  • Complex contraception
  • Complex GU
  • Psychosexual
  • IUS for non-contraceptive purposes

Psychosexual Referrals

Patients can self-refer into our psychosexual service. Please advise them to call the team on 0300 124 0102 – they can put the patient through to our medical secretary, who can go through the form with them.
