If you are a professional working with children and/or adults identified as vulnerable, you can make a referral into sexual health services by downloading the form below.

General Practice Referrals
You can make a General Practice referral for specific conditions via PRISM (Sexual Healthcare (Complex) tab)
- Complex contraception
- Complex GU
- Psychosexual
- IUS for non-contraceptive purposes
We no longer accept correspondence by fax.
